Welcome to our online application form. We are happy you are choosing SelfDesign. This application will give you an opportunity to understand our program agreements and to provide us information about you and your family.

Items needed for the application

  1. BC Medical Card for each child. (please have these on hand you will need to enter the numbers)
  2. Birth Certificate to validate Child info matches Birth Certificate
  3. A-Grade supported web browser. What is an A-Grade Browser?
